Penis Lengthening (Long Penis), Penis Thickening (Large Penis)

This form of surgery includes procedures to change and improve the appearance of the male genitals. This is a relatively new field in aesthetic surgery, however along the years this type of operation has become more popular. Numerous patients feel inhibited in their sexual relations and are also distressed by situations linked to swimming or sports.

The length of the penis can vary from a few centimetres up to 15 centimetres whilst at rest. In a recent study where penile length and dimensions measured at flaccid and erect status dimensions in 80 physically normal men before and after pharmacological erection, it has been discovered that the main flaccid length was 8.8 cm. stretched length 12.4 cm. and erect length 12.9 cm. Neither patient age nor size of the flaccid penis accurately predicted erectile length. Stretched length best closely correlated with erect length. The reason why most patients want a penis enlargement is not that they feel the penis is too small when erect, but that is too short while at rest.

Sydney Plastic Surgery offers two types of genital surgery for men. A penis enlargement for men who feel their penis is too short and a thickening procedure for those who feel their penis is too thin.

The consultation for penis lengthening surgery

During the consultation Dr Barnouti will discusses any urological and sexual problems, which may require further investigation. The length and size of the penis are evaluated, also its mobility and suitability for an enlargement.

Some men feel their penis is too thin. This can often be corrected, either in combination with an extension procedure or as a separate one. Transplanting dermis or fat to the penis shaft can thicken the penis. The size of the thickened area depends on how much of the fat stays in place. As a rule of thumb, approximately 30%-50% of the injected fat stays in permanently. The final result is visible in three months. Should the patient desire extra thickness, a touch up procedure is recommended in 12 months time.

The surgery

1. Penis lengthening surgery: The extension of the penis can be anything from a few centimetres up to 25% of its length while at rest. In specific cases an extension of 4 cm is possible. How long the extension will be is not clearly evident until the time of surgery. The operation is performed under general anaesthetic or local anaesthetics with twilight sleep (sedation).

In order to extend the penis, the ligament that suspends the penis inside the body is loosened, so that part of the penis which was inside the body is ejected and ends up on the outside. Then, the erectile tissue is fastened to its new position, followed by inter positioning of fatty tissue to further elongate and stabilise the penis during erection. Finally the skin flap advanced to further elongate the skin of the penis at its base.

Erection ability is not influenced by the procedure. As long as no complications ensue, the sensitivity of the skin and the head of the penis are not altered.

2. Penile thickening procedure: Dr Barnouti uses a refined fat transplant technique. Fat is usually extracted out of the pubic area, abdomen and waist lines in a mini-liposuction procedure. It followed by fat purification. Once blood and oil removed from the fatty tissue, the resultant healthy fatty cells are ready to be injected around the penis shaft through puncture hole. Normally, 20-30ml of fat is needed to fill the whole shaft and crown of penis. Post-operative pain is not usually severe.

The procedure is performed as a day surgery. Erection ability is not influenced by the procedure.[/accordion][accordion title=”After Surgery”] Penile lengthening: Any pain after surgery is normally manageable with simple painkillers. However, the penis may swell, and erections during this time can be painful. The discomfort diminishes after the 3rd day of surgery. You will also be asked to take oral Antibiotic as a prophylactic measurement against infection.

You will have a simple dressing at the pubic area. It is extremely important to keep the area clean and to shower twice a day from the 3rd day after the procedure.
The sutures are dissolvable. The wound will be reviewed in the clinic 5-7 days after surgery.

Penile thickening Any pain after surgery is normally linked to the area where the liposuction has been performed, rather than the penis. However, the penis will swell in the first week, and erections during this time can be painful. The discomfort diminishes after the 3rd day of surgery. You will also be asked to take oral

Antibiotic as a prophylactic measurement against infection.

You will have simple dressing at the pubic area. It is extremely important to keep the area clean and to shower twice a day from the 3rd day after the procedure.

The sutures are dissolvable. The wound will be reviewed in the clinic in a week’s time.

Getting you back to work and Normal activities

You will need rest for the first 24- 48 hours after that light duties can be commenced.

You will be able to return back to work after few days.

Patients should wait 4-6 weeks before sexual activity is resumed.

Penile Girth

The other procedure to widen and thicken the penis is what is known as “dermal fat grafting”. The dermal fat graft is harvested from the buttock, thigh, abdomen or pubic area. The graft is placed along the shaft of the penis.

Dermal Fat Grafts (DFG ) have been the gold standard of penile widening surgery and often used in combination with Phalloplasty procedure. The dermal graft is permeant.

What is the difference between using fat injections and dermal fat grafts to widen the penis?

Fat transfers by injection often have excellent initial results, but at the end of 6 weeks , 30% of the fat injected is reabsorbed into the body. Some patients require top ups in 12 months after the initial surgery. The other side effect of fat injection is “lumpy appearance”. This can be settled by massage or simple drainage. Dr Barnouti will select the surgery that suit the individual patient.  The preferred technique is dermal fat grafting.
