Lipofilling or Fat injection Surgery

Structural micro fat grafting (fat injection) is a technique that uses your body fat as a natural, living filler to achieve precise structural alterations wherever it is placed.

Fat injection reduces wrinkles and creases and plumps up areas that have lost volume due to surgery, injury, ageing, or other causes. Transplanted fat that originates from the patient himself or herself does not carry a risk of allergic reaction or rejection. Further advantages include short recovery time, no scars, and usually long-lasting results if the fat injection procedure has been done correctly.

Structural microfat grafting can also be used to correct many other problems, such as hemifacial atrophy, issues caused by previous liposuction, and cleft palate. Fat grafting is presently being used as an ancillary procedure for a facelift, brow lift, blepharoplasty, rhinoplasty, eyelid surgery, forehead lift, and breast augmentation.

Sydney Plastic Surgery offers lipo-filling (fat injection) using the “structural micro fat grafting technique” to the following areas: Face, Brow, Upper Eyelids and temples, Chin and jawline, nasolabial folds / Marionette Grooves, Cheekbones and eyelids, Lip Augmentation, Neck, Correction of Acne scarring, Body, Hands, and Liposuction deformities.

Face

Face Fat Injection

The core reasons for enhancing facial structure are to replenish atrophied tissues and to enhance facial elements or proportion.
As we age and lose the plumpness of youth, our faces get smaller. The skin remains the same size; however, without the underlying structure, it retreats into wrinkles. The wrinkles will diminish as the lost structure under the skin is restored with fat injection.

Enhancing facial proportion

With ageing, the cheeks, chin, jaw, forehead, etc., reduce in size. The fat, muscle and connective tissues atrophy or melt away, leaving behind unsupported skin. As the support underlying the skin vanishes, the skin falls into wrinkles and folds. As the cheek becomes smaller, bags under the eyes become more prominent. As the jawline and chin lose their prominence, jowls appear.

Dr Barnouti MBBS, FRACS (Plas) pays specific attention to volumetric restoration of the face and body with living filler (your own fat), correcting asymmetry and restoring the fullness of youth, naturally and subtly.

Nano fat grafting

Nanofat does not has the capacity to build up a significant fat volume. The Indications for nanofat injection are different when compared with microfat grafting.

Usually, nanofat grafting is combined with other modalities such as sharp needle intradermal fat grafting (SNIF) to obtain a soft-tissue filling effect, where the nanofat is layered more fanwise in an intradermal level to enhance the skin quality. In some areas the results can take between 1-3 months to become evident, other areas such as dark circles of the eyes, a prolonged erythematous phase, can also sometimes be expected. Nano fat grafting is recommended for  facial wrinkle lines, lower eyelids, upper eyelids hallowing, brow, lips, perioral lines, and neck line. It can also be used for cheek and temporal hallowing area in combination with micro fat grafting.

Mandatory Surgical Warning

Surgical Warning: Any surgical or invasive procedure carries risks. Before proceeding, you should seek a second opinion from an appropriately qualified health practitioner. Please read our Risks and Complications of Plastic Surgery page.

GP Referral & Assessment Requirements

In accordance with the latest April 2026 AHPRA and Medical Board of Australia regulations, all patients seeking a consultation for cosmetic surgery must obtain a referral from their General Practitioner (GP). This ensures your medical history is reviewed by an independent doctor who can discuss your motivations and goals before you meet with a specialist.

A mandatory seven-day cooling-off period applies after your initial consultation before surgery can be booked. Patients are also encouraged to seek a second opinion from an appropriately qualified health practitioner before proceeding. These requirements exist to protect patients and to ensure every decision is fully informed and free of pressure.
